Occupational Therapist (OT) – Full-Time & PRN

Meadowview Rehabilitation & Nursing Center – Lafayette Hill, PA

Meadowview Rehabilitation & Nursing Center is seeking compassionate and motivated Occupational Therapists (OTs) to join our in-house rehabilitation team! We currently have opportunities available for both Full-Time and PRN Occupational Therapists .

Position Highlights

• Full-Time and PRN opportunities available

• Skilled Nursing Facility (SNF) & Long-Term Care (LTC) setting

• In-house rehabilitation team

• Collaborative interdisciplinary environment

• Resident-centered approach to therapy

• Net Health and PointClickCare (PCC)

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans of care

• Provide skilled OT services to short-term rehabilitation and long-term care residents

• Help residents improve independence with activities of daily living (ADLs)

• Assess functional abilities, cognition, safety, and adaptive equipment needs

• Collaborate with PT, SLP, nursing, physicians, families, and other members of the interdisciplinary team

• Educate residents and caregivers on safety strategies, therapeutic exercises, and adaptive techniques

• Complete accurate and timely documentation using Net Health and PointClickCare (PCC)

• Maintain compliance with facility, state, and federal guidelines

Qualifications

• Graduate of an accredited Occupational Therapy program

• Current Pennsylvania Occupational Therapist license in good standing

• SNF/LTC experience preferred, but not required

• Strong clinical, communication, and organizational skills

• Net Health and PCC experience is a plus

• New graduates are encouraged to apply
