Job Summary

We are seeking a dedicated and compassionate Occupational Therapist to join a Skilled Nursing Facility. The Occupational Therapist will evaluate patients, develop and implement individualized treatment plans, provide skilled occupational therapy services, supervise Occupational Therapy Assistants, document patient progress, and collaborate with the interdisciplinary care team to achieve optimal patient outcomes.

Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical needs and goals.
• Provide skilled occupational therapy services to geriatric patients.
• Monitor patient progress and modify treatment plans as needed.
• Supervise Occupational Therapy Assistants (OTAs) and ensure quality of care.
• Maintain accurate and timely clinical documentation in accordance with facility, state, and federal regulations.
• Collaborate with physicians, nursing staff, rehabilitation team members, patients, and families to ensure coordinated care.
• Educate patients and caregivers on treatment plans, adaptive techniques, and equipment use.
• Participate in interdisciplinary team meetings and care planning activities.
• Maintain compliance with facility policies, regulatory requirements, and professional standards.

Education, Licensure, and Experience

Required:

• Active New Jersey Occupational Therapist (OT) license.
• Graduation from an accredited Occupational Therapy program.
• Strong communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary team.

Preferred:

• Previous Skilled Nursing Facility (SNF) experience.
• Experience working with geriatric populations.

• Full-time.
• Day shift.
• No on-call requirements.
