Pay: $61–$65/hour (based on experience) + Sign-on Bonus (if applicable) + Benefits

Job Summary

We are seeking a compassionate and licensed Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will provide individualized therapy services to residents in a Skilled Nursing Facility (SNF) and Assisted/Independent Living community, helping them improve mobility, restore function, and enhance their quality of life.

-> Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility training.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate patients, families, and caregivers on treatment goals and home exercise programs.
• Collaborate with physicians, nurses, and interdisciplinary team members to ensure high-quality patient care.
• Complete accurate and timely clinical documentation.
• Maintain compliance with all company policies, state regulations, and professional standards.

-> Qualifications

• Active California Physical Therapist (PT) License.
• Graduate of an accredited Physical Therapy program.
• Experience in Skilled Nursing (SNF), Post-Acute Care, Rehabilitation, Home Health, Assisted Living, or Long-Term Care preferred.
• Strong clinical assessment, communication, and documentation skills.
• BLS/CPR certification preferred.
• Ability to work independently while delivering compassionate, patient-centered care.
