Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 1 full year in Adult Home Health or Hospice experience, depending on the specialty. License: Must have a current nursing license. Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase EMR and OASIS within the last 6 months. - Reliable transportation with a current driver's license and required liability insurance. - Ability to travel up to 75 miles from the home clinic. - Acceptance of all patients on the schedule, regardless of distance, without declines. - Completion of all documentation within 24 hours of patient appointments. Description: This position involves coordinating and directing the provision of nursing care in patients’ homes according to physician orders. The role requires the application of the nursing process based on agency policies and procedures. The Registered Nurse is responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses. The position requires home visits and attendance at periodic office meetings. Candidates must be comfortable with travel requirements, which may include drives longer than 20 minutes between patients. Each contractor will manage a caseload of 5-10 patients per day, with productivity expectations set weekly. Types of cases include Start of Care, Wound Care, and Follow-Up Visits. The position may require participation in weekend and on-call rotations. Candidates must fully understand that there are no guaranteed hours with this contract. Each clinician must meet weekly productivity points to ensure financial viability. All travelers are expected to be proficient in the specified EMR systems.
