Community Health Navigator & Advocate
Care New England
Care New England is seeking a Community Health Worker Apprentice to join the community health team. The role focuses on home and community-based outreach, care coordination, and patient advocacy to improve health outcomes for patients with complex medical and social needs. You will conduct home visits, help patients access resources, track activities, and collaborate with nurses, physicians, and other care team members to support patient-centered care. Bilingual Spanish proficiency is required. #J-18808-Ljbffr
