About The Role The Social Worker plays a critical role in supporting individuals and families facing a variety of challenges by providing comprehensive social services and mental health support. This position involves assessing client needs, developing and implementing care plans, and coordinating with healthcare and community resources to promote well-being and stability. The Social Worker will engage in crisis intervention to address urgent situations, ensuring client safety and access to necessary services. Through empathetic counseling and case management, the role aims to empower clients to overcome obstacles and improve their quality of life. Ultimately, the Social Worker contributes to building stronger communities by advocating for vulnerable populations and facilitating access to essential resources. Minimum Qualifications Bachelor’s degree in Social Work (BSW) or a related human services field. Valid state licensure or certification as a Social Worker (e.g., LSW, LMSW) as required by the state of employment. Demonstrated experience in crisis intervention and case management within a healthcare or social services setting. Strong knowledge of mental health counseling principles and social service systems. Excellent communication and interpersonal skills with the ability to work effectively with diverse populations. Preferred Qualifications Master’s degree in Social Work (MSW) or related field. Licensure as a Licensed Clinical Social Worker (LCSW) or equivalent advanced certification. Experience working with specific populations such as children, elderly, or individuals with substance use disorders. Training in specialized therapeutic techniques or trauma-informed care. Familiarity with electronic health records (EHR) and case management software. Responsibilities Conduct thorough assessments of clients’ social, emotional, and mental health needs to develop individualized care plans. Provide crisis intervention and support to clients experiencing emergencies or acute distress. Coordinate with healthcare providers, social service agencies, and community organizations to connect clients with appropriate resources. Offer psychotherapy and counseling services to support mental health and emotional stability. Maintain detailed case records and documentation to track client progress and ensure compliance with regulatory standards. Advocate for clients’ rights and access to services within healthcare and social service systems. Participate in multidisciplinary team meetings to discuss client care and develop collaborative strategies. Educate clients and their families about available resources and coping strategies to promote long-term well-being. Skills The required skills such as crisis intervention and case management are essential for responding promptly and effectively to client emergencies and coordinating comprehensive care plans. Human services and social services knowledge enable the Social Worker to navigate complex systems and connect clients with appropriate resources. Psychotherapy and mental health counseling skills are applied daily to provide emotional support and therapeutic interventions that promote client resilience. Working within healthcare settings requires collaboration with medical professionals and understanding of healthcare protocols to ensure holistic client care.
