Care Manager I - Non-Clinical (Remote/Field, NC)

Community Care Physician Network LLC

A community-based health organization is seeking a Care Manager 1 - Non-Clinical in North Carolina to provide integrated care management for Medicaid members. This role involves assessing, planning, and coordinating health care for a diverse population. Applicants should have a related Bachelor's degree and preferably two years of experience with child welfare.
