Position: Care Coordination RN Location: Hays, Kansas Pay: starting at $57,061 (based on experience) | $500 SIGN-ON BONUS! What You'll Do:
- Help patients successfully transition from inpatient psychiatric units, hospitals, and residential programs back into community care
- Work closely with providers, nurses, and community partners to keep treatment plans aligned and effective
- Improve patient safety by reviewing medications and coordinating follow-up care
- Educate and support patients so they understand their care plan and feel confident moving forward
- Build relationships across healthcare systems that strengthen mental health care in our region
- Organized and proactive, with strong follow-through

- Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing. Preferred- Bachelor of Science in Nursing (BSN).
- Preferred: experience in psychiatric/mental health nursing, or physician office or outpatient practice.
- Preferred: experience in care coordination or case management.
- Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers.
- Ability to communicate effectively with patients and families about treatment plans and care transitions.
- Demonstrated proficiency with technology including computers, electronic records, and common office equipment.
- Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes.
