RN Field Care Manager

Humana is seeking an RN Field Care Manager to deliver in-home, member-focused care for diverse populations. In this role, you will perform comprehensive assessments, create individualized care plans, and coordinate services across physicians, community resources, and Humana’s integrated programs. You’ll provide health coaching, medication and condition education, and support safe care transitions, using data and clinical insight to improve quality and outcomes.
