Hospice Transitional Care Nurse - Inpatient & Home Discharge
Longleaf Hospice, LLC
A local hospice care provider in Atlanta is seeking a Transitional Care Nurse to provide direct nursing care and education to patients and families. Responsibilities include assessing patients, coordinating care, and educating both patients and hospital staff. The successful candidate will be a licensed registered nurse with at least five years of nursing experience, preferably including hospice or home care.
