Establish and administer a treatment program with specific goals determined according to patient capacity and tolerance under physician direction. Set realistic and achievable goals for patients. Document and record patient condition. Educate patients and families in appropriate physical therapy methods. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
