Job Summary:

We are seeking a dedicated and compassionate Occupational Therapist to join our team at Hertford Rehabilitation and Healthcare Center in Chesapeake, Virginia. As an OT, you will play a vital role in providing high-quality care to our residents, focusing on their rehabilitation, independence, and overall well-being. Responsibilities:

• Provide comprehensive occupational therapy evaluations, assessments, and treatment plans for residents in accordance with medical orders and facility policies

• Document patient progress and data accurately and in a timely manner

• Provide individualized therapy sessions to residents, utilizing a variety of modalities and techniques

• Collaborate with the interdisciplinary team to develop and implement effective care plans

• Participate in staff meetings, education sessions, and in-service training as needed

• Maintain accurate and up-to-date records of patient care and therapy sessions

• Adhere to facility policies and procedures, including those related to patient confidentiality and Federal Resident Rights

• Demonstrate a commitment to excellence in patient care and professionalism in all interactions

Requirements:

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy

• SNF experience preferred

• Current license and/or registration in the state (as applicable)

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Compassion, empathy, and a strong patient-centered approach
