Linley Park Post Acute is a beautiful 88-bed skilled nursing and rehabilitation facility located in the heart of Anderson, SC. Position Summary

We are seeking a licensed Occupational Therapist (OTR) to provide high-quality, patient-centered therapy services. This role includes evaluation, treatment planning, team collaboration, and supervision of therapy staff, all focused on improving residents' independence and quality of life.

Key Responsibilities

• Evaluate residents within required timeframes and develop individualized treatment plans
• Provide therapy services per physician orders and established goals
• Document evaluations, treatments, and progress notes in compliance with regulations
• Supervise COTAs, aides, and students in accordance with state practice act
• Collaborate with the interdisciplinary team on care planning and patient progress
• Participate in care conferences, utilization reviews, and rehab meetings
• Educate residents, families, and caregivers on therapy programs and discharge plans
• Assist with durable medical equipment (DME) recommendations
• Provide staff training and participate in in-services
• Ensure compliance with all state, federal, and payer requirements

Qualifications

• Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred)
• Active South Carolina Occupational Therapist (OTR) license in good standing
• Experience in skilled nursing or rehab setting preferred
• Familiarity with PointClickCare and Casamba preferred
• Strong clinical, documentation, and leadership skills
