Compensation

Salary Range: $212,944 annually $269,216 annually

What You Can Expect

The role of the Physician in a community setting is a response to societys demand for accessible, quality health care for all populations, regardless of socio-economic status.

Individual accountabilities and work volume will be established through the development of annual Success Objectives within the framework outlined below.

This position requires the ability and willingness to work some nights and weekend shifts as required to align with community health needs. The position may also require participation in mobile health initiatives.

Key Responsibilities

Patient Care and Clinical Practice

  • Facilitate entry of the patient into the health care system by assessing primary health care needs
  • Elicit comprehensive health histories
  • Perform physical examinations
  • Order and/or perform pertinent diagnostic tests
  • Analyze collected data to determine client health status and formulate a problem list/diagnosis
  • Provide coordinated, continuous, and culturally competent primary health care to clients across the health continuum through mutual goal setting and care planning
  • Prescribe a variety of treatment modalities to meet health and illness needs
  • Manage and evaluate the plan of care, process, and outcomes, then modify the plan as needed
  • Coordinate services with other team members to meet patients primary health care needs and ensure continuity of care
  • Document comprehensive, accurate, and continual data in client records and program reports according to Organization standards

Collaboration and Team-Based Care

  • Collaborate with advanced practice nurses, community health nurses, and other medical providers to ensure health teaching and counseling is provided
  • Collaborate with multidisciplinary professionals and community agencies to provide high-quality care
  • Practice collaboratively with advanced practice nurses and community health nurses to prevent duplication of services and meet Organization productivity requirements
  • Consult with health team members regarding client treatment plans
  • Provide clinical supervision of medical care delivered by Physician Assistants, PA-Cs
  • Serve as a collaborating physician to Advance Practice Nurses, APNs
  • Follow clinical practice standards, protocols, and responsibilities related to each health care providers role

Productivity, Access, and Continuity of Care

  • Meet average monthly provider productivity standards, including but not limited to number of visits
  • Follow role assignments aligned with Patient Centered Medical Home standards
  • Participate in all Patient Centered Medical Home efforts
  • Assure access and continuity of care by providing call for after-hours coverage
  • Assure continuity of care by obtaining hospital privileges at designated hospitals
  • Develop and maintain a patient panel as clinical competence is achieved to enhance continuity of care and meet Organization productivity standards

Community and Professional Outreach

  • Seek opportunities for community and professional outreach to maintain and increase the patient referral base

Outreach activities may include, but are not limited to:

  • Providing training and presentations on health care issues to multidisciplinary professionals and community agencies
  • Attending community health fairs
  • Disseminating information about the health center and services provided to other agencies or health care professionals

Research, Education, and Professional Development

  • Incorporate and disseminate research findings as a basis for practice and to improve health care outcomes
  • Assume responsibility for maintaining competence in clinical practice by
    • Engaging in mentor relationships with experienced mid-level providers and physicians to gain clinical expertise
    • Participating in appropriate educational activities to maintain Board certification and continue professional growth

Legal, Ethical, and Professional Standards

  • Practice within the legal and ethical guidelines established by the Medical Practice Act, the Physicians Code of Ethics, and Specialty Standards of Practice
  • Adhere to professional standards as outlined by governmental bodies, the American Medical Association, AMA, the American College of Obstetrics and Gynecology, ACOG, the American College of Physicians, the American Academy of Pediatrics, the American Academy of Family Physicians, the American Academy of Nurse Practitioners, the American Academy of Physician Assistants, private funding sources, Organization plans/policies, and Unit guidelines
  • Participate in periodic evaluative reviews and/or in-house and external staff training to ensure continued understanding of and adherence to applicable standards

What You Will Bring to Howard Brown Health

Education and Experience

  • Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree, or accredited by the American Osteopathic Association with a Doctor of Osteopathy degree
  • If medical education occurred at a foreign medical school, the applicant must possess a current Educational Council Foreign Medical Graduate, ECFMG, certificate
  • Successful completion of an approved residency program
  • Experience establishing and maintaining effective working relationships with other employees, patients, and the general public

Knowledge

  • Knowledge of the principles and practice of medicine
  • Knowledge of the structure and function of community health centers

Language Skills

  • Ability to read, analyze, and interpret the most complex documents
  • Ability to communicate effectively with patients and their families

Mathematical Skills

  • Ability to work with mathematical concepts such as probability and statistical inference
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations
  • Ability to work with fundamentals of plane and solid geometry and trigonometry

Reasoning Ability

  • Ability to define problems, collect data, establish facts, and draw valid conclusions
  • Ability to interpret technical instructions in mathematical or diagram form
  • Ability to work through situations involving several abstract and concrete variables

Certificates, Licenses, and Registrations

  • Active Illinois Physician license
  • Current DEA registration
  • Board certification in a specialty area, or ability to complete within two years of employment
  • Documentation of Medical Degree from an accredited school of Medicine
  • Documentation of residency completion in a specialty area from an accredited residency program
  • Physician National Data Bank verification of legal claims history
