Case Manager

Position Summary The Division of Community Behavioral Health (CBH), within the Department of Psychiatry & Behavioral Sciences, is seeking a self-motivated individual to join our dynamic multi-disciplinary team. The mission of CBH is to address behavioral health care disparities through: health services research and evaluation; training and workforce development; and through increasing access to quality behavioral health services that are holistic, cost-effective and provided with respect to unique cultures and communities. CBH is seeking a dedicated and compassionate Case Manager to join our Early First Episode Psychosis (FEP) Program and broader early psychosis initiatives. This program supports young adults experiencing a first episode of psychosis, as well as their families, with a focus on recovery, resilience, and community reintegration. As part of a multidisciplinary Coordinated Specialty Care (CSC) team, the Case Manager will work collaboratively with clinicians, peer support specialists, and vocational providers to help clients achieve greater independence. Key focus areas include housing, benefits and health insurance, financial literacy, transportation, care coordination, education, employment, recreation, and social integration. Primary Responsibilities Provide client-centered case management services in both community and clinic settings. Conduct intake assessments to identify needs, strengths, and barriers. Collaborate with clients to develop individualized recovery goals using shared decision-making. Offer direct support to improve life skills, access resources, and navigate systems. Maintain thorough and timely documentation in accordance with program standards. Participate actively in weekly team meetings and ongoing interdisciplinary collaboration. Receive training in the Supported Education and Employment (SEE) model to assist clients in pursuing academic and vocational goals as part of their recovery. Support the program’s outreach and engagement efforts, including: Updating and distributing educational materials Scheduling and conducting outreach presentations Assisting with provider training and community education statewide Vital Qualifications Strong commitment to person-centered, recovery-oriented care Excellent communication, organizational, and interpersonal skills Ability to work independently and as part of a collaborative team Compassion, empathy, cultural humility, and self-awareness This is a unique opportunity to make a meaningful impact in the lives of young people navigating early psychosis while gaining specialized experience in a nationally recognized evidence-based model. This is a full-time, term, benefits eligible position. We are hopeful that this position will continue beyond the listed term. This position may require limited travel within NM, including to meet with clients. This position is based in Albuquerque. This position will work mostly in-office and clinics, and remotely on occasion. Conditions of Employment Successful candidate must submit to post-offer, pre-employment physical examination and medical history check. Specialty licensure/certification may be required, as specified by the department. Minimum Qualifications Bachelor's degree; at least 1 year of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
