What You'll Do

As a Home Health Physical Therapist, you will:

Evaluate and treat homebound patients referred for physical therapy services
Develop individualized treatment plans designed to maximize patient independence and function
Assess strength, balance, mobility, endurance, gait, pain, and activities of daily living
Deliver evidence-based therapeutic interventions and exercises
Collaborate with physicians, nurses, and interdisciplinary care teams
Educate patients and caregivers to support recovery and long-term success
Monitor progress and adjust treatment plans as needed

Qualifications

Graduate of an accredited Physical Therapy program (Bachelor's or Entry-Level Master's Degree)
Active Connecticut Physical Therapist License
CPR Certification (required by start date)
Prior Physical Therapy experience
