Work Environment

This role is primarily focused on providing therapy in patients' homes, allowing you to work in a variety of environments that range from well-familiarized family spaces to tailored therapeutic settings. By establishing in-home care routines, you will make a significant impact on patients' recovery processes while working alongside supportive family members, caregivers, and other health professionals. Our onsite focus means you will have the opportunity to directly engage with patients in their own environments, carefully assessing and adapting treatments to fit their lifestyles.

Key Responsibilities

• Evaluate patients in their home settings to assess their physical therapy needs and develop customized treatment plans based on individual evaluations.
• Implement treatment strategies utilizing a comprehensive range of modalities, including therapeutic exercises, manual therapy techniques, and assistive devices aimed at reducing pain and improving mobility.
• Continuously track patient progress, documenting detailed notes on treatment responses and adjusting plans as needed to meet evolving therapeutic goals.
• Educate patients and their families on rehabilitative practices, including the use of assistive devices, exercises, and methods to ensure continuity of care and success in recovery.
• Participate in ongoing professional development through workshops, certifications, and peer networking to remain current with best practices in physical therapy.

Patient Population / Client Focus

Your caseload will encompass a diverse range of individuals, from children recovering from surgery to older adults managing chronic illness or mobility issues. You will have the opportunity to work closely with each patient to develop trust and adapt interventions that promote independence and improve their overall quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Current, valid state license to practice physical therapy in California.
• Excellent written and verbal communication skills to engage effectively with patients, families, and interdisciplinary team members.
• Proficiency in word processing and computer skills, enabling accurate documentation and reporting of patient information.
• Flexibility and adaptability to respond to the changing needs of your patients and their families.
• No prior experience is required; however, a passion for patient care and willingness to learn is essential.

Preferred Experience

• Experience in home health therapy settings is advantageous, but not mandatory.
• Exposure to diverse patient demographics and therapy techniques through internships or clinical rotations.
• Background in working collaboratively within a health care team is a plus.
