If you are a skilled CNA who values meaningful, patient-centered care, this is your opportunity to make a difference!How You'll Make a Difference (Essential Functions)The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.Key ResponsibilitiesAssist Patients with Activities of Daily LivingAssist with ambulation, changing position, and moving to/from sitting and standing.Help with bathroom functions like toileting and incontinence care.Help bathe, shower, dress, provide oral care, and groom.Assist with eating and drinking.Perform post mortem care.Obtain vital signs. Assist with treatments.Anticipate and respond to patient needs.Provide Homemaker AssistancePerform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash.Order and deliver patient supplies.Help prepare simple meals.CommunicationRead, understand, and follow the plan of care.
