Infusion RN - Per Diem - Land O' Lakes

Job Summary Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety and compliance under a physician’s plan of care. Responsibilities Initiate, develop, and implement intermediate nursing plan of care treatments, evaluating patient progress toward goals. Organize and participate in the provision of direct patient care, perform treatments, administer medications, and educate patients and families/caregivers. Modify plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assess patient needs and physical status at each skilled visit through health data access and patient interview; re-evaluate needs through reassessment and supplemental orders. Obtain and clarify physician orders for plan of treatment revisions, inform physicians promptly of significant changes, and provide written summaries. Provide training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Communicate effectively and timely with Option Care’s clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepare clinical documentation in real‑time during visits and submit to the nursing department to comply with established timelines for billing optimization. Coordinate discharge planning and prepare discharge summaries with patient instructions, reporting care needs, progress, and goals when transferring care. Demonstrate compliance with agency operations, policies, professional standards, local, state, federal regulations, and accreditation standards. Maximize work efficiency through the use of computers and automation technologies to validate treatment orders and communicate patient care. Respond promptly and appropriately to patient requests and initiate emergency procedures as necessary. Accept accountability for own practice and maintain ethical and professional conduct. Maintain confidentiality of patient information in accordance with HIPAA regulations. Explain Option Care’s scope of services and financial obligations for patients. Attend required training modules, in‑services, and continuing education to maintain licensure and competency. Provide oversight and coordination of paraprofessionals in the home setting and, if applicable, act as a supervisor in the absence of the supervisor or nurse manager. Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses during orientation, onboarding, and supervisory activities. Participate in nursing department on‑call responsibilities and perform other related duties as directed by the supervisor. Insert PICC/Midlines per physician order; troubleshoot and declote obstructed central line catheters; monitor PICC/Midlines and educate on infection prevention and routine care. Education & Experience Active and unrestricted RN license in the state of practice. Minimum of 1 year of nursing experience. Current CPR certification. Advanced certification and training applicable to infusion nursing (preferred). Bachelor of Science in Nursing (preferred). 1–2 years of previous infusion nursing experience (preferred). Experience operating a motor vehicle in the state of practice; access to a vehicle for business travel and proof of liability insurance. Basic knowledge of computer operating systems and software applications. Physical Demands Ability to lift up to 50 pounds (maximum 75 pounds); exert force up to 20 pounds occasionally or 10 pounds frequently. Work may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying. Fine motor skills and visual acuity required: close‑ and distance vision, color and peripheral vision, depth perception, focus adjustment. Potential exposure to blood, body fluids, other infectious materials, hazardous substances, needles, sharps, fumes, chemicals, humidity, temperature extremes, adverse weather, animals, second‑hand smoke/vape, and unpredictable home environments. Safety requirements: closed‑toe shoes, facemask, goggles, gown and/or gloves, and functional communication device for routine and emergency situations. Travel Requirements Willingness to travel up to 100% of the time for business purposes.
