Physical Therapist (PT) – Skilled Nursing Facility
Location: Gloversville, NY 12078
Job Type: Contract
Duration: 12 Weeks
Shift: Day | 8:00 AM–4:30 PM
Schedule: Monday–Friday
Local Candidates: Required
Traveler Candidates: Required
Pay Rate:
Local: $57–$63/hr, based on experience

Required Qualifications:
- Current and active New York Physical Therapist (PT) license — Required
- BLS certification — Required
- Previous Skilled Nursing Facility (SNF) experience — Required
- Experience providing physical therapy services to nursing home residents
- Strong patient assessment and treatment-planning skills
- Strong clinical documentation skills
- Ability to work independently and as part of a multidisciplinary team
- Strong communication and interpersonal skills
Responsibilities:
- Evaluate residents and develop individualized physical therapy treatment plans
- Provide physical therapy services in a Skilled Nursing Facility/Nursing Home setting
- Perform therapeutic exercises, mobility training, functional activities, and other appropriate interventions
- Monitor patient progress and modify treatment plans based on clinical needs
- Assist patients with mobility, strength, balance, and functional independence
- Educate patients, families, and caregivers regarding treatment plans and safety techniques
- Maintain accurate and timely clinical documentation
- Collaborate with nursing staff, physicians, occupational therapists, and other healthcare professionals
- Follow facility policies, infection-control procedures, patient-safety standards, and regulatory requirements
- Provide patient-centered care throughout the assignment
