Registered Nurse - Long Term Care

Responsibilities:

• Provide high-quality, patient-centered care to our long-term care residents, including administering medications, wound care, and IV therapy

• Develop and implement individualized care plans in collaboration with the healthcare team

• Perform routine vital signs, monitor and report changes in resident condition to the healthcare team

• Participate in resident assessments, care plan development, and care coordination

• Collaborate with interdisciplinary teams to ensure seamless transitions and optimal resident outcomes

• Maintain accurate and thorough records of resident care

• Actively participate in quality improvement initiatives to enhance the resident experience

Requirements:

• Current RN licensure in the state of Illinois
