The Home Health Nurse plays a vital role in delivering comprehensive patient care within a small team environment, typically consisting of 2-5 nurses. Reporting directly to the Nurse Manager, this position involves conducting patient assessments, coordinating care, managing medications, and educating patients primarily in chronic disease management and post-surgical care. The role requires regular local travel and on-call availability to ensure continuous patient support and optimal health outcomes.

Responsibilities

• Conduct comprehensive patient assessments
• Coordinate care among healthcare providers and family members
• Manage and administer medications accurately
• Provide patient education on disease management and care plans
• Maintain detailed and accurate documentation
• Communicate effectively with patients, families, and the care team
• Travel locally to patient homes and respond to on-call needs

 

Preferred Qualifications

• 1+ years of nursing experience
• Registered Nurse (RN) license
• CPR certification
• License Practical Nurse (LPN)
• Home Health Aide experience
• Proficiency in patient assessment and wound care
• Experience with medication administration and care planning
• Ability to monitor vital signs and implement infection control protocols
• Familiarity with Electronic Medical Records (EMR) systems
• Strong communication, time management, and empathy skills
