Join a dedicated care transitions team as a full-time Advocate Social Worker (BSW), providing compassionate patient and family support through psychosocial interventions. This role focuses on promoting wellness, improving care outcomes, and facilitating access to post-acute physician services and community resources within a regional healthcare setting in the Midwest.

Job Summary

Collaborate with interdisciplinary teams to assess and address the psychosocial needs of patients referred to the care transition program. Conduct home visits to evaluate social, cultural, emotional, and spiritual factors affecting patients and their families. Develop and implement tailored intervention plans that incorporate these aspects to enhance patient care and reduce hospital readmissions.

Key Responsibilities

• Gather comprehensive psychosocial data and perform assessments for patients and families.
• Work closely with care management staff and healthcare providers to coordinate timely and effective services.
• Address barriers such as financial or social challenges that may impact medical compliance and recovery.
• Respond appropriately to cases involving suspected violence, abuse, or neglect, adhering to professional ethics.
• Facilitate advance care planning, including discussions on healthcare proxies, living wills, and power of attorney.
• Maintain accurate and timely documentation in compliance with privacy regulations.
• Engage in continuous learning to stay current with clinical trends, regulatory changes, and best practices.

Education and Experience

• Completion of an accredited Bachelor of Social Work (BSW) program is required.
• Preferred: Several years of experience in medical facilities, hospice, or home care environments.

Licenses and Certifications

• Current licensure as a BSW in the applicable state.
• Healthcare Provider Basic Life Support (BLS) certification.
