Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be part of a growing team providing the highest quality care for patients in SNF settings. In this role, you will evaluate and treat patients following an individualized plan of care. Responsibilities include: Providing excellent care for our patients Delivering excellent customer service to patients and family members Collaborating with facility staff to ensure optimal patient care Evaluating new patients, developing plans of care, and setting measurable, functional goals Treating patients, addressing concerns, and educating them on their injury, progress, and probable outcomes Supervising Physical Therapist Assistants Re-evaluating patient progress, revising plans of care, and updating goals Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes #J-18808-Ljbffr Enhance Rehab
