Registered Nurse Case Manager

How you’ll make an impact in this role

• Assesses, plans, and coordinates multidisciplinary inpatient care to optimize clinical outcomes, manage length of stay, and prevent avoidable readmissions.

• Conducts utilization reviews and communicates with third-party payors to secure authorizations and ensure appropriate level-of-care status.

• Evaluates post-acute care needs and arranges durable medical equipment, home health, skilled nursing placements, or community resources.

• Serves as the primary patient and family advocate, facilitating discharge planning, goals-of-care discussions, and smooth care transitions.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
