Case Manager Rn – Acute Hospital

Shift: 5x8 Days, with shift times between 07:00–17:00 Weekends: Yes, required every other weekend Guaranteed Hours: 72 hours per 2 weeks License: Current RN license in the state required Experience: Minimum of 1 year acute care hospital experience required EMR: Epic experience required Certifications: ACM certification preferred; BLS preferred Skills: Discharge planning, care coordination, utilization review; familiarity with InterQual and Milliman criteria; basic ICD-10 familiarity Coordinate discharge planning and care for diverse patient populations, including neonates, pediatrics, adults, and geriatrics. Collaborate with interdisciplinary teams including physical therapy, pharmacy, social services, and others involved in patient care. Conduct utilization review and continued stay reviews to ensure appropriate patient care and resource management. Maintain thorough documentation within the electronic medical record system. AK RN license in hand required for Auto Offer; pending license may be acceptable for submission Only 1 time-off request per 13-week assignment; no holiday RTO for Auto Offer Willingness to work 2 of these 3 holidays — Thanksgiving, Christmas, New Year Max 1 calendar year at facility, then 6-month break before returning
