Social Worker - Case Management

RN – Social Work

Schedule: 4x10 Day Shift

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 72 hours per 2-week period

Requirements:

• License: Washington clinical license required
• Certifications: BLS certification
• Years of Experience: Minimum 1 year clinical experience
• EMR: Experience with EPIC
• Unit-specific skills:
• Discharge planning and care coordination
• Short-term acute inpatient social work or case management
• Experience with adult and geriatric populations
• Interdisciplinary coordination

Roles & Responsibilities:

• Manage patient discharge planning and care coordination
• Provide case management services for adult and geriatric populations
• Collaborate with an interdisciplinary team to facilitate patient care
• Effectively manage a variable caseload and complex discharges

Compliance / Notes:

• Compliance with weekend and holiday work requirements
• Orientation duration: 40 hours
• Facility-approved dress code: Any scrubs color except reddish-orange
• Background checks are required
