About the Role:

Responsibilities:

• Conduct initial in-home client assessments.
• Develop individualized care plans based on client needs.
• Perform supervisory visits and reassessments as required.
• Review and update care plans when client needs change.
• Ensure compliance with Georgia home care regulations and agency standards.
• Collaborate with clients, families, caregivers, and the Administrator.
• Provide caregiver education and guidance regarding client care needs.
• Document all assessments, supervisory visits, and care plan updates accurately and timely.
• Report concerns regarding client safety, health changes, or quality of care.
• Participate in quality assurance and regulatory compliance activities.
• Provide high-quality nursing care to patients on an as-needed basis.
• Educate patients and families on health management and wellness.
• Adhere to all safety and infection control protocols.

Requirements:

• Valid nursing license in the state of Georgia.
• Previous nursing experience
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-centered approach to care.
• Proficient in electronic health record systems.
