Rather than bedside or facility work, you'll support patients remotely across the full arc of their care. Untangling complicated health situations, moving them toward their wellness goals, and knocking down the Social Determinants of Health (SDOH) that get in their way. You control your calendar. Full-time, part-time, or somewhere in the middle. The role is entirely remote. You'll connect with patients by video or phone, never in person. Your Day-to-Day - Lean on your LPN/LVN clinical background while getting fluent in Solace's proprietary platform, tools, and standards. - Turn empathy and attentive listening into patient relationships built on genuine trust. - Read each patient's situation, sort out what matters most first, and shape a care plan that keeps things moving over time. - Design care plans that go beyond the clinical, factoring in everyday barriers like access to food, getting to appointments, and support in the home. Who We're Looking For - A current LPN or LVN license in good standing. - A track record of working with patients across many different backgrounds and communities. - Compassion: your LPN/LVN experience is rooted in caring about people and standing up for those who can't stand up for themselves. - Steadiness and structure: you keep multiple priorities organized and hold it together when the pace picks up. - Quick technical footing: unfamiliar systems don't intimidate you — you get comfortable fast. - Ownership: you spot what needs handling and handle it, without waiting for a nudge.
