Clinical Care Coordinator

Providence Health and Services Anchorage, AK Full Time Job Title Clinical Care Coordinator Responsibilities The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. Key duties include: coordinating services for panel patients, especially those who face difficulty navigating the healthcare system; managing specialty referrals; supporting patients in self‑care and identifying relevant community resources; collaborating with and outreaching to outside service providers to provide seamless transitions of care; and providing patient education while assisting with access to applicable resources.
