Position SummaryWe are seeking an experienced National Medical Director – Occupational Health to provide clinical leadership and physician oversight for our occupational health programs nationwide.
This physician will serve as the senior clinical authority for the program and must maintain an active, unrestricted physician license in all 50 states , or otherwise possess the licensure necessary to provide the required nationwide clinical oversight.
The National Medical Director will oversee clinical activities performed by nurses, Nurse Practitioners (NPs), Physician Assistants (PAs), and other clinical staff; provide physician oversight for laboratory testing and orders; review clinical documentation; make or oversee pre-employment medical determinations; and serve as the supervising/collaborating physician where required.
The role will work closely with clinical operations and leadership to ensure services are delivered consistently, efficiently, and in accordance with applicable federal and state requirements.
Key ResponsibilitiesNational Clinical Oversight- Serve as the senior physician leader for occupational health services nationwide.
- Provide clinical oversight and guidance to nurses and other clinical team members.
- Establish and maintain clinical standards, protocols, standing orders, and escalation procedures.
- Provide clinical consultation for complex or unusual occupational health cases.
- Support quality assurance and clinical compliance initiatives across the organization.
- Serve as the ordering or overseeing physician for laboratory testing when physician authorization is required.
- Review and approve appropriate laboratory orders, protocols, and standing orders.
- Provide physician review of abnormal, critical, or clinically significant laboratory findings when necessary.
- Ensure laboratory ordering and review processes comply with applicable clinical and regulatory requirements.
- Review medical histories, examination findings, laboratory results, and supporting medical documentation.
- Make or oversee pre-employment medical and fitness-for-duty determinations .
- Provide physician-level review when a candidate or employee requires additional medical clearance.
- Review cases escalated by nursing or clinical staff.
- Provide medical determinations and recommendations consistent with the essential functions and medical requirements of the applicable position.
- Support return-to-work and other occupational health determinations as required.
- Review physician, NP, PA, and nursing notes for clinical appropriateness, completeness, and quality.
- Provide secondary physician review of cases requiring escalation.
- Identify documentation or clinical quality concerns and provide feedback to clinical staff.
- Assist in developing standardized documentation and clinical review processes.
- Serve as a supervising, collaborating, or oversight physician for Nurse Practitioners and Physician Assistants where required by state law and organizational structure .
- Review and sign clinical documentation when required.
- Maintain appropriate supervisory or collaborative agreements.
- Provide clinical consultation and escalation support to advanced practice providers.
- Ensure NP/PA practice remains within applicable scope-of-practice and supervisory requirements in each state.
- Provide physician-level clinical leadership to occupational health nursing teams.
- Develop and approve nursing protocols and standing orders.
- Provide escalation pathways for cases requiring physician review.
- Support clinical training, competency, and quality standards for nursing staff.
- Maintain knowledge of federal and state requirements affecting occupational health services and clinical practice.
- Partner with compliance, legal, operations, and clinical leadership to maintain appropriate clinical policies and procedures.
- Participate in clinical audits and quality reviews.
- Identify clinical risks and implement corrective actions when necessary.
- Maintain appropriate documentation supporting physician oversight and clinical decision-making.
- MD or DO from an accredited medical school.
- Board certification in Occupational Medicine, Family Medicine, Internal Medicine, Emergency Medicine, or another relevant specialty preferred.
- Active and unrestricted physician licensure in all 50 U.S. states required , or ability to obtain and maintain the licensure required to perform the responsibilities of the position nationally.
- Significant experience in occupational health, employee health, urgent care, workforce medicine, or a related clinical environment.
- Experience supervising or providing clinical oversight to NPs, PAs, nurses, and other clinical professionals .
- Experience reviewing laboratory results, clinical documentation, and medical clearance cases.
- Strong understanding of state-specific physician supervision and scope-of-practice requirements.
- Experience making pre-employment, fitness-for-duty, return-to-work, or similar occupational health determinations preferred.
- Strong clinical judgment with the ability to make consistent decisions across a high-volume national program.
- Excellent communication, leadership, and documentation skills.

- NP/PA supervision across multiple states
The National Medical Director will provide the physician leadership necessary to support a consistent, compliant, and scalable national occupational health program . This individual will serve as the primary physician resource for clinical escalations, laboratory oversight, advanced practice provider supervision, nursing protocols, documentation review, and employment-related medical determinations across the organization.
