Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5 days a week (Monday - Friday), from 08:00 to 16:30.

Job Description

The Transplant Coordinator will coordinate all patient care needs before, during, and after transplantation within the hematology/oncology specialty. The role requires substantial experience in hematology and oncology, particularly regarding pre-transplant evaluations and assessments. The Coordinator will manage multiple patient cases simultaneously and ensure compliance with transplant center protocols.

Responsibilities

• Coordinate pre-transplant evaluations and assessments of candidates.
• Interpret lab results and diagnostic tests related to transplantation.
• Monitor vital signs, weight, fluid balance, and graft function.
• Review and interpret relevant diagnostic and laboratory data.
• Ensure patient confidentiality in accordance with HIPAA regulations.
• Educate patients and families regarding transplant processes, including lifestyle adjustments and medication adherence.
• Schedule and track all pre- and post-transplant testing according to required timelines.
• Collaborate with the transplant team and assist with organ offers, acceptance, and recipient preparation.
• Provide psychosocial support and education for post-transplant care.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in hematology/oncology.
• BLS certification required.
• Chemo certification (ONS/ONCC) preferred.
• Experience as a patient navigator or case manager is a plus.
