Licensed Practical Nurse (LPN) needed to work with individuals with developmental disabilities.  This position is for weekends (Friday, Saturday and Sunday, including any holiday that may fall on a scheduled work day), 30 hours per week, full-time.  Some of the duties include, but not limited to:

a. Enters medication administered into Therap, and check for accuracy.

b. Ensure proper storage and destruction of all medications.

c. Monitor and document skin integrity. Perform preventative measures for skin breakdown under supervision of RN.

d. Ongoing assessment of health and safety needs of the individuals.

e. Review orders from physicians regarding medical care and implement as required, with RN approval.

f. Track (bowel, seizure, food, fluid, blood pressure, blood sugar and others) as required.

g. Assessment of Care, documented in Therap, subject to RN review and approval.

h. On Site Observation.

i. Assist RN with monthly review of care, documentation, medication regiment and physicians orders.

j. Other duties as assigned.
