Licensed Practical Nurse (LPN) – Long-Term Care

Location: Montrose, NY 10548
Schedule: Night Shift — 5×8 hours, 11:00 PM–7:30 AM
Duration: 26 weeks

Job Overview

We are seeking a qualified Licensed Practical Nurse (LPN) to provide compassionate nursing care and support to residents in a long-term care setting . The ideal candidate will have experience caring for geriatric patients and be comfortable working independently during overnight shifts.

Key Responsibilities

• Provide nursing care and support to residents in a long-term care environment.
• Communicate effectively with residents, address concerns, and encourage participation in treatment and therapeutic activities.
• Provide compassionate, resident-centered care to the geriatric population .
• Monitor residents and promptly report changes in condition or other concerns to the appropriate healthcare team.
• Maintain adequate levels of medical and resident-care supplies.
• Complete supply requisitions and assist with inventory management.
• Help maintain a clean, safe, and sanitary resident-care environment.
• Provide post-mortem care in accordance with established procedures when required.
• Follow applicable facility policies, nursing standards, and safety procedures.

Required Qualifications

• Active New York State LPN License .
• Minimum 2 years of LPN experience .
• At least 1 year of post-licensure clinical nursing experience preferred .
• Previous Long-Term Care/Geriatric experience preferred .
• Experience working with the Veteran population preferred .
• Strong communication, observation, and resident-care skills.
• Ability to work effectively during overnight shifts.

Education

• High School Diploma required.
