Certified Nursing Assistant (CNA)

Job Summary:

The Certified Nursing Assistant (CNA) in Long Term Care/Transitional Care settings provides direct patient care and support under the supervision of nursing staff. The role involves assisting patients with daily living activities, monitoring patient status, and collaborating with the healthcare team to ensure quality care. This position requires at least one year of skilled long term care or transitional care experience and BCLS certification. Responsibilities:

• Assist patients with routine activities of daily living including hygiene, mobility, feeding, and skin care.
• Perform range of motion exercises, ambulate patients, and provide positioning support.
• Collect patient data and specimens such as urine, stool, and sputum; record vital signs and pain assessments.
• Communicate patient status and report changes to licensed nursing staff.
• Assist with patient transport, preparation for diagnostic tests, and rehab therapy support.
• Maintain and clean equipment, prepare rooms, and provide post mortem care as needed.
• Perform clerical duties including answering unit phones, data entry, and coordinating equipment/supply needs.
• Float between units including Transitional Care Unit and other areas as assigned.

Required Skills & Certifications:

• Valid Certified Nursing Assistant (CNA) license.
• Current BCLS (Basic Cardiac Life Support) certification.
• Minimum of 1 year experience in skilled Long Term Care or Transitional Care settings.
• Reliable transportation for travel between units.

Preferred Skills & Certifications:

• Experience working in multiple care units or floating between departments.
• Familiarity with electronic charting systems such as Net Solutions.

Special Considerations:

• Must work every other weekend as scheduled.
• No on-call requirements.
• Floating between units required, typically spending three shifts per 24 hours in Transitional Care Unit with remaining shifts in other areas.
• Reliable personal transportation is mandatory due to floating and travel between units.
• Business casual attire required.
• Facility may call off up to one shift per pay period.

Scheduling:

• 8-hour day shifts.
• Block scheduling accommodations available.
• Weekend work required every other weekend.
