Job Description Requirements: Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred. Active licensure as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required. Minimum of 3 years of recent acute hospital case management experience, with at least 2 years in a leadership or management role. Strong knowledge of care coordination, discharge planning, utilization management, and regulatory compliance. Demonstrated leadership, communication, and change management skills. Ability to manage multiple priorities, analyze performance data, and collaborate across interdisciplinary teams.

The Director of Case Management provides strategic and operational leadership for hospital case management and social work services. This role oversees daily operations, staffing, and performance to ensure high-quality, patient-centered care coordination and efficient resource utilization. The Director partners with executive leadership and interdisciplinary teams to align case management practices with organizational goals, regulatory standards, and quality benchmarks while fostering a culture of accountability, collaboration, and continuous improvement.
