Serving North Royalton, Broadview Heights, Brecksville, and Strongsville, Ohio

Help patients recover, understand their care, and feel more confident at home. Elara Caring is seeking a Registered Nurse to provide home health care throughout Southern Cuyahoga County .

You’ll build relationships with patients and families, use your clinical judgment, and coordinate care around each patient’s needs. Home health experience is preferred, but not required , making this an opportunity to bring your nursing skills to a new setting.

Make a Difference During Every Visit

• Complete initial and ongoing assessments to identify patient needs.
• Provide nursing care according to individualized care plans and physician orders.
• Monitor patient progress, report changes, and take appropriate action.
• Teach patients and families how to manage care, recognize concerns, and stay safe at home.
• Communicate with physicians and the care team to support coordinated, continuous care.
• Maintain accurate clinical documentation and protect patient and staff privacy.
• Support safe, quality care through timely follow-up and clinical judgment.

What You’ll Need

• Current RN license valid in Ohio.
• Graduation from an accredited certificate, diploma, associate, or bachelor’s nursing program.
• At least 1 year of nursing experience in a clinical care setting .
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
