As our Home Health services continue to grow in Pueblo, this is an exciting opportunity to support patients in their homes while helping build and strengthen care in the community.Pay range: $30–$34 per hour, based on experienceAbout the RoleAs a Physical Therapist Assistant, you will provide skilled therapy services under the supervision of a licensed Physical Therapist. You will play an important role in helping patients improve mobility, function, and overall quality of life through individualized, patient-centered care delivered in the home setting.Key ResponsibilitiesProvide physical therapy services in accordance with established treatment plans and under the supervision of a licensed Physical TherapistComplete visit documentation accurately and in a timely mannerReview patient diagnoses and conditions, observing all indications, contraindications, and required precautionsCommunicate patient progress, concerns, and changes in status promptly to the supervising Physical TherapistWhy Sangre de Cristo Community Care?
