Responsibilities include: Evaluate patients and develop individualized treatment plans Deliver skilled physical therapy services in the home setting Build meaningful patient relationships through one‑on‑one care Complete documentation accurately and on time Coordinate schedules and communicate with the clinical support team Drive clinical initiatives focused on delivering exceptional patient outcomes We’re looking for clinicians who are confident, compassionate, organized, and motivated by independence and patient impact. Qualifications Active New Jersey Physical Therapist License Medicare documentation and billing experience preferred Strong communication and time management skills Comfortable working independently Computer literacy Current CPR Certification Professional liability insurance Ability to wear PPE as needed Valid Driver's License Geriatrics Orthopedics Home Health Physical & Rehabilitation Medicine This role is built for Physical Therapists who value flexibility, independence, and meaningful patient relationships.
