Job Title: PACU RN Profession: Registered Nurse Specialty: Post Anesthesia Care Unit Duration: 13 weeks Shift: 12H Nights Hours per Shift: 34.5 hours per week Experience: 2 years License: Required Certifications: BLS Certification, BLS (AHA) Must-Have: FLU vaccine required - no exemptions or exceptions ICU background and acuity experience in PACU as boarders Pediatric experience required Strong indicators: SICU, Transplant ICU, Neuro ICU, or mixed-acuity ICU Management of vents, vasoactive drips, and invasive monitoring Comfortable with rapid deterioration and escalation Prior PACU exposure is necessary Description: Candidates must have experience caring for kidney transplant patients post-operatively. Strong indicators include strict I&O and monitoring urine output trends and labs. Awareness of immunosuppression risks is essential. Ability to identify early signs of rejection, bleeding, or delayed graft function is important. Concerns to monitor in the first few hours after a kidney transplant include urine output, hemodynamics, electrolyte shifts, and bleeding or graft perfusion. Experience recovering patients after Neuro IR procedures such as thrombectomy or aneurysm coiling is required. Familiarity with frequent neuro checks and tightly controlled BP parameters is expected. Candidates should be aware of access site monitoring (femoral/radial). Acute neuro changes, decreased LOC, new weakness, severe headaches, and access site bleeding or hematoma would warrant immediate escalation of care. Knowledge of caring for bariatric patients in the post-operative setting is necessary. Candidates should mention airway risk, obstructive sleep apnea, and hypoventilation. Understanding safe mobilization and pain control mindful of respiratory depression is critical. Airway obstruction, rapid desaturation, CO2 retention, and aspiration risk are concerns in the PACU setting for bariatric patients. Candidates should be able to describe a situation where they had to manage or escalate an airway concern. Calm escalation and early recognition of issues are preferred. Familiarity with collaboration with anesthesia and preparation for CPAP/BiPAP or reintubation is necessary. Hesitation to escalate or minimizing airway issues is not acceptable. Candidates may have multiple high-acuity patients to prioritize in the PACU. Utilization of protocols and time-based reassessments is expected. Clear communication with anesthesia and surgeons is crucial. Ability to act quickly under pressure is important. Candidates should provide examples demonstrating clear thinking, ownership of decisions, and appropriate escalation. Reflective learning afterward is desired. Effective communication of concerns to anesthesia or surgery when something seems wrong is required.
