Position Overview

As a Licensed Practical Nurse (LPN), you will provide direct resident care, monitor health and vital signs, maintain accurate documentation, and work closely with Registered Nurses (RNs) and Unit Managers to ensure residents receive individualized, high-quality care. You will also help foster a positive, professional environment for the entire care team.

Key Responsibilities

• Deliver direct personal care to residents and monitor overall health and well-being

• Observe, assess, and record resident vital signs and changes in condition

• Collaborate with RNs and Unit Managers to implement and maintain care plans

• Maintain accurate and timely documentation in compliance with company and state regulations

• Provide support, guidance, and training to residents, families, and team members as needed

• Promote a safe, professional, and compassionate environment for residents and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license in Massachusetts

• Long-term care experience preferred but not required

• Strong communication, teamwork, and organizational skills

• Compassionate, dependable, and dedicated to resident-centered care

• Ability to work flexible schedules across all shifts

Shifts & Positions

• All shifts available: 7am3pm, 3pm11pm, 11pm7am

• Full-Time, Part-Time, and Per Diem opportunities
