LPN Geriatric Care

Job Summary:

The Licensed Vocational Nurse (LPN) will provide direct nursing care to geriatric long-term care residents, focusing on medication administration and oversight of CNA colleagues. The role requires strong critical thinking skills, the ability to work independently, and delivering care with dignity and respect. The LPN will serve residents without acute care needs, primarily providing wound care in a long-term care environment. Responsibilities:

• Provide direct patient care including medication administration.
• Oversee and collaborate with Certified Nursing Assistants (CNAs).
• Perform nursing duties appropriate to long-term care residents.
• Apply critical thinking to assess and address patient needs independently.
• Maintain respectful and dignified care for all residents.
• Document patient care and communicate effectively with the interdisciplinary team.

Required Skills & Certifications:

• Active Licensed Practical Nurse (LPN) license in New York State.
• American Heart Association Basic Life Support (BLS) certification.
• Minimum of 2 years' experience in long-term care nursing.
• Strong critical thinking and independent work skills.

Preferred Skills & Certifications:

• Experience with wound care management in a long-term care setting.
• Previous supervisory experience overseeing CNAs.

Special Considerations:

• Work includes rotating day, evening, and night shifts.
• Must be available to work weekends, holidays, on-call, and callback as needed.
• Care environment: geriatric long-term care without acute care or tracheostomy patients.

Scheduling:

• Rotating shifts covering day, evening, and night.
• Shift assignments based on unit needs, including weekends and holidays.
