The Hospice Skilled Nurse provides compassionate and comprehensive care to patients with life-limiting illnesses, managing symptoms and supporting families throughout the end-of-life process. This role involves frequent travel between patient homes and close collaboration with an interdisciplinary team including physicians, social workers, chaplains, and therapists. Nurses typically manage a caseload of 1-5 patients, ensuring individualized care plans and thorough documentation to maintain quality and regulatory compliance. We are especially seeking bilingual Spanish-speaking LVN and RN candidates to join our dedicated team.

Responsibilities

• Provide direct patient care and symptom management
• Develop and implement personalized care plans
• Assess patient status regularly and document findings accurately
• Collaborate with physicians, social workers, chaplains, and therapists
• Communicate effectively with patients’ families and caregivers
• Support patients and families through end-of-life care and decision-making
• Travel frequently between patient homes to deliver care
• Monitor compliance with hospice care regulations and standards

Required Qualifications

• Minimum 1 year of experience in hospice or skilled nursing

Preferred Qualifications

• Active RN license
• Active LVN license
• BLS/CPR certification
• Strong patient assessment and palliative care skills
• Excellent communication and time management abilities
• Compassionate care approach and effective team collaboration
• Bilingual in Spanish preferred
