Duties

Provide comprehensive primary care and preventative care to an established panel of primary care patients, under the Patient Aligned Care Team (PACT) model. The type of care is served to the Veteran population with diseases consistent to those seen in the general population include but are not limited to the following: acute and chronic illness, women's health, gender specific, mental health, and preventive care, along with coordination of care delivery. A major focus is on preventative medicine.

Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare. Participate in weekly team meetings and provide innovative management ideas for patient case reviews.

Duties include, but are not limited to, history taking, physical examination, ordering appropriate labs, diagnostic tests, and consultations needed.

Formulate multi-disciplinary treatment plans.

Perform medication reconciliation.

Document all medical information in an electronic medical record.

Other duties as assigned.
