This role is essential for providing clinical guidance and identifying educational opportunities to enhance overall patient care.Job RequirementsEducation and Work Experience:Associate's/Technical Degree in nursing or equivalent combination of education and related experience: RequiredThree years' experience in Home Care: Preferred Licenses/Certifications: Current Registered Nurse (RN), Licensed Vocational Nurse (LVN), or Licensed Practical Nurse (LPN) in the state of practice: RequiredEssential FunctionsTimely receipt and documentation of new referrals.Coordinate care with the patient and interdisciplinary team.Engage with physician offices and community liaisons to assess and process referrals and admissions for service.Communicate effectively with patients, team members, healthcare professionals, and referral sources.Provide assistance and information to community members regarding home and community-based needs.Address patient and agency needs professionally and creatively.Verify patient insurance and explain benefit information to patients and case managers.Manage incoming calls for patient intake and assist with overflow as needed.Maintain confidentiality of information in accordance with HIPAA guidelines and regulations.Distribute referrals to appropriate clinicians with essential background information and care plans.Work independently, meet deadlines, and manage multiple tasks effectively.Foster strong communication with all referral sources.Participate in various clinical tasks, including faxing, scanning, emailing, and copying.Perform other job-related duties as assigned.Organizational RequirementsWe prioritize the safety and wellbeing of our associates and patients.
