Monitor and Document Resident Conditions: Observe residents, accurately document findings, and communicate changes in condition to the appropriate healthcare team members. Support Care Planning and Admission Processes: Collaborate with RNs and interdisciplinary teams in developing, implementing, and updating individualized care plans, including during admissions and readmissions. Participate in and Oversee Resident Care Programs: Actively support resident care initiatives (e.g., skin integrity, pain management) and ensure CNAs fulfill their responsibilities within these programs. Nurse Requirements Active Wisconsin Registered Nurse (RN) or Licensed Practical Nurse (LPN) license Current CPR certification Long-term care experience preferred Basic computer skills, including electronic health records systems Ability to push/pull up to 60 lbs. and occasionally lift up to 100 lbs.
