Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Evaluate and assess patient needs for case management and care coordination.
• Provide comprehensive discharge planning and utilization management.
• Collaborate with interdisciplinary teams to ensure effective patient care transitions.
• Complete initial assessments and develop care plans for diverse populations.
• Utilize EMR systems such as Interqual and Cerner for patient documentation.
• Participate in weekend rotations, working at least one full weekend per month.

Qualifications

• Active RN license in the state of California.
• Bachelor of Science in Nursing (BSN) required.
• Certifications: BLS Certification.
• Experience with Interqual criteria and discharge planning.
• Prior experience in case management within a hospital setting.
• Strong skills in communication, collaboration, and reprioritization.
