Job overview

The  Certified Home Health Aide (CHHA)provides compassionate, non-medical personal care and supportive services to clients in their homes under the supervision of a Registered Nurse (RN). The CHHA assists clients with activities of daily living (ADLs) and instrumental activities of daily living (IADLs), helping them remain safe, comfortable, and as independent as possible while maintaining their dignity and quality of life.

Primary Responsibilities

• Assist with personal care, including bathing, dressing, grooming, oral hygiene, toileting, and skin care.
• Assist with mobility, ambulation, transfers, positioning, and range-of-motion (ROM) exercises as outlined in the Plan of Care.
• Prepare meals and assist with feeding and hydration as needed.
• Provide medication reminders (no medication administration).
• Perform light housekeeping, laundry, linen changes, grocery shopping, and meal preparation.
• Escort clients to appointments and assist with errands when authorized.
• Observe and promptly report changes in the client's physical, emotional, or mental condition to the RN Supervisor.
• Accurately complete required documentation, visit notes, and time records.
• Follow the individualized Plan of Care and agency policies and procedures.
• Maintain client confidentiality and comply with HIPAA regulations.
• Practice proper infection prevention and standard precautions.
• Promote a safe home environment and report any safety concerns immediately.
• Treat all clients and families with dignity, compassion, respect, and professionalism.

Education and Experience

Qualifications

• Current New Jersey Certified Home Health Aide (CHHA) certification.
• Ability to communicate effectively in English. Bilingual or multilingual candidates who speak Spanish, Haitian Creole, or other languages are strongly encouraged to apply.
• Ability to lift, transfer, and assist clients safely.
• Ability to work independently while following the RN Supervisor's direction.

Experience

• Previous experience as a  Certified Home Health Aide (CHHA), Personal Care Assistant (PCA), Certified Nursing Assistant (CNA), or caregiver is preferred but not required.
• New Jersey Certified Home Health Aides who have successfully completed a state-approved training program are encouraged to apply.
• Experience providing care to seniors, adults with disabilities, or individuals with chronic illnesses in a home care, assisted living, nursing home, rehabilitation, or hospital setting is preferred.
• Experience assisting clients with activities of daily living (ADLs), instrumental activities of daily living (IADLs), mobility assistance, meal preparation, companionship, and light housekeeping.
• Experience caring for clients with dementia, Alzheimer's disease, Parkinson's disease, post-surgical needs, hospice support, or other chronic medical conditions is a plus.
• Ability to accurately document care provided and promptly report changes in a client's condition to the Registered Nurse (RN) Supervisor.
• Demonstrated ability to provide compassionate, dependable, and client-centered care while maintaining professionalism, confidentiality, and respect for client rights.
• Ability to work independently in clients' homes while following the individualized Plan of Care and agency policies and procedures.

Knowledge of Languages (Preferred)

• Ability to communicate effectively in English.
• Bilingual or multilingual candidates who speak  Spanish, Haitian Creole, or other languages are highly valued to better serve our diverse client population.

Physical Requirements

The CHHA must be able to:

• Stand, walk, bend, kneel, stoop, and reach for extended periods.
• Lift, transfer, push, or pull up to 50 pounds with or without reasonable accommodation.
• Assist clients with mobility, transfers, and repositioning using proper body mechanics and assistive devices when appropriate.
• Travel to clients' homes as assigned.

Primary Job Duties

The Certified Home Health Aide (CHHA) provides safe, compassionate, high-quality personal care and supportive services to clients in accordance with the individualized Plan of Care and under the supervision of the Registered Nurse (RN).

The CHHA's primary duties include:

• Provide compassionate, respectful, and client-centered care.
• Communicate effectively with clients, families, supervisors, and other healthcare professionals.
• Read, write, speak, and understand English. Bilingual candidates who speak  Spanish, Haitian Creole, or other languages are encouraged to apply.
• Follow written and verbal instructions and accurately carry out the client's individualized Plan of Care.
• Provide personal care, including bathing, dressing, grooming, toileting, oral hygiene, skin care, and hair care.
• Assist clients with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Assist with ambulation, transfers, positioning, range-of-motion (ROM) exercises, and the use of assistive devices.
• Assist with meal planning, meal preparation, feeding, and encouraging adequate hydration.
• Provide medication reminders in accordance with the Plan of Care (no medication administration).
• Perform light housekeeping duties, including laundry, linen changes, washing dishes, vacuuming, dusting, and maintaining a clean and safe environment.
• Assist with grocery shopping, errands, and transportation to appointments when authorized.
• Provide companionship, emotional support, and social interaction while promoting client dignity and independence.
• Observe, document, and immediately report any changes in the client's physical, mental, emotional, or environmental condition to the Registered Nurse (RN) Supervisor.
• Follow the client's individualized Plan of Care and perform only those duties authorized by the agency and permitted under New Jersey regulations.
• Maintain accurate, complete, and timely documentation of all services provided.
• Practice proper hand hygiene, infection prevention, standard precautions, and appropriate use of personal protective equipment (PPE).
• Maintain client confidentiality and comply with HIPAA, agency policies, and all applicable federal and New Jersey regulations.
• Maintain a safe home environment by identifying and reporting potential hazards or safety concerns.
• Attend required orientation, in-service education, competency evaluations, and staff meetings.
• Demonstrate professionalism, reliability, punctuality, respect, integrity, and compassion in all interactions.
• Wear the required agency uniform and identification badge while providing client care.
• Maintain regular communication with the Clinical Coordinator and Registered Nurse Supervisor regarding client care needs, scheduling concerns, incidents, or emergencies.
• Perform other duties within the CHHA scope of practice as assigned by the Registered Nurse Supervisor or Director of Nursing.
• Caring for clients with chronic illnesses, dementia, Alzheimer's disease, Parkinson's disease, disabilities, and those recovering from illness or surgery.
• Proper documentation of care provided, visit notes, and agency-required forms.
• Respecting clients' cultural, religious, and personal preferences while providing individualized care.
• Understanding and complying with all applicable New Jersey Home Care Service Firm (HCSF) regulations and DT Healthcare of New Jersey's policies and procedures.
