Social Worker - Respiratory and Rehabilitation Center in Coventry

Job Summary:

We are seeking a compassionate and motivated Social Worker to join our team at Respiratory and Rehabilitation Center in Coventry. As a Social Worker, you will provide emotional, social, and practical support to residents and their families, while working collaboratively with our interdisciplinary team to ensure the best possible resident experience.

Responsibilities:

• Provide social services support to residents and families

• Assist with admission, adjustment, transfer, and discharge planning

• Support residents with emotional and psychosocial needs

• Coordinate with nursing, therapy, administration, and outside agencies

• Maintain accurate and timely documentation

• Help resolve resident and family concerns in a professional and caring manner

• Ensure compliance with all facility policies and applicable regulations

Qualifications:

• Previous experience as a Social Worker in a healthcare or long-term care setting preferred

• Strong communication and interpersonal skills

• Compassionate, organized, and team-oriented

• Ability to manage multiple tasks and work effectively with residents, families, and staff

• Knowledge of discharge planning and resident rights preferred
