RN - Hospice

Registered Nurse

The Registered Nurse (RN) will provide direct patient care services to hospice patients. Supportive care will also be provided to the patient's family. The RN will manage an interdisciplinary group, including Licensed Practical/Vocational Nurses and Certified Nursing Assistants. The goal is to enhance the quality of hospice care provided. Assure the highest quality of care is provided and documented in accordance with the Plan of Care. Perform the initial and ongoing assessment of the patient. Assist in developing and implementing the Hospice Plan of Care as prescribed by the physician. Work closely with other members of the Interdisciplinary Group to provide all-inclusive services, including bereavement support. Report any changes in the patient's condition to both the Attending Physician and Hospice Physician. Maintain accurate and updated medical records, including all interdisciplinary notes and assessments. Practice infection control measures and standard precautions. Monitor scheduling of all staff. Participate in the Quality Assessment and Performance Improvement Program. May be requested to participate in the on-call schedule. Promote the company's core values consistently. Complete required compliance training annually. Perform other assigned activities and attend staff meetings. Maintain a high level of knowledge related to pain and symptom management in the dying process. Conduct routine supervisory visits with all team members at least monthly. Perform ongoing review and follow-ups on assigned patient clinical records to ensure quality service. Maintain close contact with the patient's family to provide information, support, and continuity of care. Act as the primary liaison between the physician, patient/family, and the hospice team. Be fully aware of all clinical issues, staffing, and scheduling concerns. Responsible for educating the patient, family, or caregivers as identified on the Plan of Care. Prepare written instructions for patient care. Make a supervisory visit to the patient's residence at least monthly to assess the aide's competence in providing care.
